Program Position: Nurse Family Partnership (NFP) – Nurse Home Visitor
Reports to: Nurse Supervisor
Job Specification: Nurse Family Partnership (1.0 FTE). This is a salaried and exempt position.
Salary: $63,000.00 This is a Grant funded position that is likely to be renewed . Summary: Nurse-Family Partnership (NFP) is an evidence-based home visiting program that supports parents beginning in early pregnancy through their child’s second birthday. NFP aims to positively impact and transform the lives of first-time mothers and their babies through a proven model that connects families with specially trained home-visiting nurses. These nurses provide guidance, education, and support during pregnancy and the early years of a child’s life.
The NFP Nurse Home Visitor delivers comprehensive nursing services to eligible women and their families. This role requires adherence to the highest standards of clinical nursing practice and strict compliance with the NFP model, as well as the policies, procedures, and standards of both NFP and the lead agency. The Nurse Home Visitor reports directly to the NFP Nurse Supervisor.
Responsibilities Basic Job Functions - Provides home visits to women and their families eligible for the NFP Program with caseload of 25 clients.
- Adheres to nursing process and the NFP model of home visitation.
- Understands, supports, represents, and coaches others in the Nurse-Family Partnership vision, mission, and model in actions and verbally to both internal and external customers and colleagues
- Assists in creating a positive work environment that promotes productivity, mentoring, teamwork, cooperation, and consideration of differing viewpoints
- Develops therapeutic relationships, utilize concepts of reflection and motivational interviewing with women and their families in a home visiting environment.
- Assesses physical, emotional, social and environmental needs of women and their families as they relate to the NFP domains.
- Assists women and their families in establishing goals and outcomes.
- Provides education, support and referral resources in assisting women and their families in attaining their targeted goals.
Administrative - Supports policies, procedures, guidelines and standards of NFP and the lead agency.
- As requested/required, participates in community outreach and promoting public awareness in support of NFP.
- Participates in agency events.
- Completes all required NFP education and training.
- Actively engages in professional development to meet all NFP Nurse Home Visitor competency requirements.
- Complete a minimum of three outreach activities per calendar year, such as attending community events, presenting to partner organizations, or engaging in local health initiatives.
Clinical - Meet with Nurse-Family Partnership nurse supervisor weekly for one-to-one reflective practice supervisory sessions
- Schedule joint home visits with Nurse-Family Partnership nurse supervisor every four months
- Attend and participate in bi-weekly case conferences and team meetings
- Participate in review and analysis of reports for achievements and areas for improvement
- Participate in quality improvement efforts
- Maintain confidentiality and adhere to HIPAA regulations.
- Document according to nursing standards and agency policy appropriately and in a timely manner.
- Perform related duties as assigned or required by the agency
Qualifications and Education Requirements - Covid-19 vaccination is required as a condition of employment
- Current unencumbered NC or SC RN license required
- BSN required
- Current CPR required
- Excellent written and verbal communication skills
- Valid driver's license and insured automobile required
- Must be able to work a flexible schedule including some evenings and weekends
- Bilingual Spanish Preferred-If hired as Spanish speaking candidate, employee must be fluent in understanding, speaking, reading, and writing in Spanish
- Two years recent experience in maternal/child health, public health, mental/behavioral nursing, or diverse communities preferred
- Home visiting experience preferred
- Able to travel by air to Denver, CO for face-to-face education sessions when required (1 week for initial training)
