Requirements:

• Active, unencumbered Registered Nurse (RN) license.
• Current Healthcare Provider BLS certification.
• Minimum of three (3) years of clinical experience as an RN, preferably with a focus on diabetes care and/or chronic disease management.
• Certification as a Diabetes Educator (CDE/CDCES).
• Bachelor’s degree in Nursing (BSN).
• Prior patient education or teaching experience.
• Experience with electronic health record systems.

Responsibilities:

• Conduct initial and annual diabetic foot assessments, including review of diabetes history and medications.
• Assess foot structure, circulation, skin integrity, and nail condition; accurately document findings.
• Provide routine foot care, including:
  • Nail trimming and filing.
  • Reduction of thickened nails
  • Management of calluses, corns, and ingrown nail portions (within scope of practice).
• Inspect and evaluate patient footwear for proper fit and safety.
• Identify abnormalities or complications and refer patients to appropriate medical providers as needed.
• Educate patients on proper foot care, prevention strategies, and available support resources.
• Maintain accurate, timely documentation in the electronic health record (EHR).
• Collaborate with interdisciplinary healthcare teams to support optimal patient outcomes.
